  • Your primary care physician's name and phone number
  • Your referring physician's name and phone number (if different than your primary care physician)
  • Your insurance information

Please don't hesitate to call our office if you have questions before your appointment, and we'll do our very best to assist you.

The day of your appointment...

Please arrive early for your appointment to give our staff time to get your paperwork processed for your visit. Your first visit with the doctor usually takes 45 minutes to an hour, but you may be in our office up to 1-2 hours total including registration and bloodtests afterwards if you need them.

Don't forget to stop at the reception desk to check out after your visit with your physician; our friendly staff will help you make your next appointment and help you get orders for any additional tests the doctor may order.

After your visit...

Make sure you create an account with our Patient Portal for secure access to your personal health record (PHR). It's an easy way to stay connected to your health information.
